Syndrome associant anemie hemolytique microangiopathique, thrombopenie et atteinte d'organe. Includes SHU et PTT.

Signes de Gravite

Anemie hemolytique (schizocytes au frottis)
Thrombopenie
Insuffisance renale
Troubles neurologiques (confusion, convulsions)
Fievre

Prise en Charge IAO

Bilan

PAS, PAD, FC, T°C, SpO2
NFS avec frottis (recherche schizocytes)
Reticulocytes, LDH, bilirubine libre
Test de Coombs direct negatif
Iono, uree, creat, LDH
Haptoglobine effondree

Orientation

SAUV toujours

Traitement

Transfusion

Transfusion si Hb < 7-8 g/dL (culots rouges de preference)
Echanges plasmatiques si PTT (plasmaphaerese)

Cas specifiques

SHU a E. coli : antibiotiques, pas d'antibiotiques anti-motilite
PTT : echange plasmatique urgent, defibrotide en cours d'evaluation
SHU atypique : eculizumab

Scope, PANI
Diurese, bilan heodinamique

Orientation

SAUV pour initiation du traitement
Reanimation selon etat clinique
