Otites

Definition

Inflammation de l'oreille moyenne d'origine virale ou non. Association d'une otalgie intense pulsatile avec diminution de l'audition du côté atteint dans un contexte fébrile.

• Box

Prise en Charge Médicale

Bilan

• Aucun si diagnostic évident
• Otoscopie : otite congestive (tympan érythémateux restant transparent), otite purulente (tympan opaque et bombant), otite otorrhéique (écoulement purulent par l'oreille ; tympan inexaminable)

Traitement

Otite congestive

• Pas d'antibiotique
• Antalgique : paracétamol 1 g x 3/j
• Pas d'AINS ni de corticoïdes

Otite purulente

• Amoxicilline (Clamoxyl®) PO : 1 g x 3/j pendant 5 jours
• En cas d'allergie aux pénicillines : céfuroxime axétil (Zinnat®) (C2G) : 250 mg x 2/j pendant 5 jours

Otite otorrhéique

• Antalgique : paracétamol (1 g x 3/j)
• Pas d'AINS ni de corticoïdes
• Si otorrhée : Oflocet auriculaire® : 1 instillation matin et soir pendant 10 jours

Syndrome otite/conjonctivite

• Amoxicilline-acide clavulanique (Augmentin®) 1 g x 3/j pendant 5 jours
• En cas d'allergie aux pénicillines : cefuroxime axétil (Zinnat®) : 250 mg x 2/j pendant 5 jours

Orientation

• Ambulatoire
• Réévaluation obligatoire à 72 h, puis 5 jours après arrêt du traitement
