Clinique

• Douleur brutale intense HCD en regard du foie
• Déclenchée par un repas copieux riche en graisses
• Irradie vers l'épaule homolatéral
• Fièvre
• Signe de Murphy +
• Diagnostic différentiel CN qui irradie vers les organes génitaux

CAT

• O2/VVP
• Spasfon 1amp en IVD
• Ou Perfalgan 1flc en IVL
• Ou Spasfon 2amp diluées dans 150cc SSI ou SG5% 30min

Traitement de sortie

• Spasfon lyoc 80mg 1lyoc 2/j
• Débridat 200cp 1cp 2/j pdt 5j
• Ou Dusphatalin gel 200mg 1gel 2/j
• Ou Spascol gel 200mg 1gel 2/j

• Si vomissements : Domperidone cp 1cp 2/j 15min avant repas
• Ou Clopramide cp

Bilan

• FNS/Gly
• CRP/VS
• ASAT/ALAT/GT/PA
• BT/BD/BI
• Lipasémie

• Échographie AP

• Douleur + Fièvre => Cholécystite
• Douleur + Fièvre + Ictère => Angiocholite

• Spasfon/8h
• Perfalgan/8h
• SSI/8h
• Primperan/12h
• ATB : Flagyl 500mg/12h
• Ciprofloxacine 200mg 2/j
• Ou Bactrim Ou Augmentin 1g 2_3/j
