Bronchiolite

Clinique

• Dyspnée expiatoire
• Toux asmathéiforme
• Râles sibilants, sifflement
• Fièvre, vomissements
• <2 ans: 1er épisode =gt; Bronchiolite
• 2ème épisode =gt; crise d asthme
• >2 ans =gt; début d asthme

Conduite à tenir

• 1ère séance de Nébulisation: 20 min
  • Salbutamol 0.03mg/kg + 3cc SSI
• Si amélioration: Traitement de sortie
• Si pas de réponse: 2ème séance de Nébulisation
• Si pas de réponse: 3ème séance + Corticoïdes
  • Celestene gtt 10gtt/kg
  • Ou Solumedrol 1mg/kg IM (max 40mg)
  • Ou HHC 5mg/kg (max 200mg) IM
  • Ou Dexa 0.6 mg

Bilan

• FNS/Gly
• Urée/Créat
• TP/CK
• Ionogramme
• TLT si sévère ou 1er épisode
• ECBU
• Constantes vitales chaque 2h: FR/FC/SaO2
• Diurèse/24h

Hospitalisation

• O2 thérapie
• Arrêt alimentation orale si vomissements
• Aspiration/4h
• Séance Nébulisation/6h
• Si fièvre: ATB
  • Claforan 100mg/kg/j toutes les 6h en IVD
  • Corticoïdes: HHC 5mg/kg/6h matin à jeûne
• Réhydratation

Traitement de sortie

• Bonne hydratation
• Fractionner les tétés
• Désobstruction nasale au SSI/2h
• Paracétamol: 15mg/kg 4/j
• Antitussifs déconseillés
