Clinique
- Fievre, sueur nocturne, douleur thoracique
- Toux seche puis purulente
- Syndrome de condensation et absence des signes ORL
- Dyspnee..
- Auscultation: Rales crepitents
- FR > 25/min
- FC > 100/min
Radio
- Opacite dense,homogene, systématisee, segmentaire ou lobaire
- voire atteignant tout un poumon
- Bronchogramme aerique
Bilan: 🩸
- FNS: Hyperleucocytose a PNN
- CRP+++
- Procalcitonine
Si Hospitalisation
Traitement:
- Sansgravite: Amoxicilline 1g 3/j
- Si non: Claraithromycine 500mg 2/j pdt 5_7j
- Ou Zeclar 15mg/kg/j 1ddp 2/j pdt 8j
- Controle radio clinique 48_72h
Enfant 👶
- Amoxicilline sirop 125/250mg 100mg/kg/j 3/j pdt 10j
- Ou Augmentin 100/250mg 80mg/kg/j 2/j pdt 10j
- Si non: Ceftriaxone 50_100mg/kg/j <2g/j
- Ou Erythromycine VO 50mg/kg/j
- Doliprane 60mg/kg/j
- Amoxicilline / Augmentin si non Macrolide
Hospitalisation si <6mois ou severe
Pneumonie severe
- Detresse respiratoire febrile
Traitement:
- Claforan inj 150mg/kg/j 3/j en IVL
- Ou Vancomycine 60mg/kg/j 3/j en IVL
- Genta inj 5mg/kg/j pdt 3_5j
- +/- Flagyl inj 15mg/kg/8h 45mg/kg/j
- Si jeune: Amoxicilline 3g/j
- Age ou comorbidite: Augmentin
